Home Care services include: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy and Home Care/Aides/Homemakers. Community Care services: vaccinations, screening programs and health promotion.

Medicare and Medicaid certified. Most insurances also accepted. Individuals without third party payers pay privately on a sliding scale fee based on income and assets. Sliding scale fees are subsidized by funding from state and county grants, United Way and donations. Per visit fees for Nurses range from $0-$120, Physical Therapist from $0-$138, Occupational Therapist from $0-$130, and Home Care Aide from $0-55. *Please note that rates are subject to change at any time.
